In order to qualify for an Enhanced Primary Care Plan, you must have at least 3 chronic conditions identified by your General Practitioner. Once this has been determined, your doctor will then provide you with a care plan that will entitle you upto 5 Medicare funded visits per calendar year, with the physiotherapist.
